Programs We Offer
CCSP (Community Care Services Program)
The Community Care Services Program (CCSP) provides in-home and community-based services to elderly and disabled individuals who would otherwise require nursing home care.
Services may include:
- Personal support

- Home-delivered meals
- Emergency response services
Eligibility Requirements:
- Must be eligible for Medicaid
- Must meet nursing home level of care
SOURCE (Service Options Using Resources in a Community Environment)
SOURCE is a coordinated care program that combines primary medical care with home and community-based services for individuals with chronic health conditions.
Services may include:
- Case management
- Skilled nursing
- Personal support services
Eligibility Requirements:
- Must have Medicaid
- Must have chronic medical conditions
- Must meet nursing home level of care
ICWP (Independent Care Waiver Program)
ICWP supports individuals with severe physical disabilities or traumatic brain injuries, helping them live independently in their homes and communities.
Services may include:
- Personal support
- Specialized medical equipment
- Home modifications
Eligibility Requirements:
- Must be Medicaid eligible
- Must have a severe physical disability or traumatic brain injury
NOW & COMP Waivers
The NOW (New Options Waiver) and COMP (Comprehensive Supports Waiver Program) provide services for individuals with intellectual and developmental disabilities (IDD/I-DD).
Services may include:
- Community living support
- Respite care
- Behavioral support services
- Day programs
